Government Records Access & Management Act Records Request

The West Valley City Recorder's Office responds to all requests for records made under the Utah Government Records Access and Management Act (GRAMA).

To request a record online:

  1. For all West Valley City Police Department related requests, complete the Online Police GRAMA Request form.
  2. For all West Valley City related requests, complete the Online GRAMA Request form.

You may also submit a request as follows:

  1. Print and complete the GRAMA Request form and fax it to 801-963-3206.
  2. Download and complete the GRAMA Request form and submit via email to nichole.camac@wvc-ut.gov.
  3. Print and complete the GRAMA Request form and submit it via mail or in person to:
